4 Lucky Dogs Adoption Application
4 Lucky Dogs Pet Rescue (4LD) is a foster-based rescue with no shelter or facility to view our dogs. Our address is a mailing address only. 4LD rescue dogs are in loving foster homes throughout West-Central Florida. For example, Hillsborough County, Pinellas County, Manatee County, and Pasco County.
Our motto is to Rescue, Rehab, Rehome, and Repeat. Pets need homes and people need pets, so our goal is to enable those matches.
This questionnaire is designed to help 4LD and yourself determine if a specific dog is right for you and your household. We are looking for the best match of a new home after evaluating and getting to know the animal through fostering him/her. We do not want the pet to have to be re-homed again.
We are an all-volunteer non-profit rescue. Our 4LD volunteers work full time, have families with responsibilities to them, and care for pets of their own (since we are pet lovers!). Please be patient with our adoption process based on our volunteer availability.

We are asking this question to match what we discovered when evaluating the animal of how the pet will react.

Rescue pets have different needs based on age, personality, or something in their past. Some will fit well with family members that are working away from the house and some will need people being at home a lot. The questions below are to determine work/home situations to help match it to specific pet.

PLANS FOR THE NEW PET
The reason we are asking the below questions is because some pets are fine by themselves without humans for periods of time, and other pets depend on people around constantly. We understand answers may depend on the specific pet such as some do need to be crated during training. These answers will help us determine the best pet for your lifestyle.
How many hours on average will the pet be alone without humans during a day (24 hours)?
Where will the new pet "hang out" during the day (7am to 10pm) and what type of activities will he/she do? If weekdays and weekends are different due to work schedules, please describe each.
What options will the new pet have of where to sleep in your house (check all that are possible)? The question is for options since animals also have preferences of their own.
How will the new pet be cared for if you go on vacation or travel out of town? For example, go with you, by relative, by neighbor, by dog sitter, boarded, etc.
Which family member will have primary responsibility for the new pet's care such as feeding and bathroom needs?
Which family member will have financial responsibility for the new pet's needs such as vet visits, prevention medications, and grooming if needed?
How much money during a year's time do you think is a reasonable amount to expect to spend on a pet's needs (vet exams, shots, heartworm/flea prevention, grooming, unexpected medical needs, etc.)?
Do you have any upcoming changes in the next two years that could affect this new pet such as moving, having a baby, or job schedule. If so, please explain. Some of the rescue dogs will adjust easily and others may not based on their past so we are comparing this question's answer to what we know about the specific dog.

By providing 4LD with these answers you are allowing 4LD to contact your vet. If you are called by 4LD for a phone interview and we mention that we need to verify vet care information, please call your vet's office and ask them to authorize the release of information to 4 Lucky Dogs.

4LD makes every effort to provide accurate history and assessment of the dog, but we are not able to guarantee the dog’s age, breed, medical status, behavior, or disposition. No one can predict how any dog will react in each situation, especially without complete knowledge of its background, as in the case with a rescued dog.
